Perioperative Management of a Patient with Hereditary Angioedema undergoing Lumbar Interbody Arthrodesis

چکیده
Hereditary angioedema (HAE) is a rare genetic disorder resulting from an inherited deficiency or dysfunction of the C1-esterase inhibitor. It is characterized by recurrent and self-limiting angioedema episodes, most often affecting the skin or the mucosal tissues of the upper respiratory and gastrointestinal tracts. Androgens and fresh frozen plasma (FFP) are frequently used as treatment despite multiple, significant side effects. Newer therapies include C1-inhibitor concentrate (C1INH) and contact system modulators such as ecallantide and icatibant. In addition, C1-INH can also be used for prophylaxis. We present the case of a 47-year-old male with a previous diagnosis of HAE, underwent elective lumbar interbody arthrodesis under general anesthesia. Following prophylaxis with danazol and C1-INH, surgery was carried out uneventfully with an unremarkable postoperative course. Perioperative prophylaxis to avoid acute attacks and anesthetic management are discussed. Central Beleña et al. MOJ Womens Health 2017, 5(4): 00129 Perioperative prophylaxis with Danazol and C1 inhibitor concentrate (C1-INH) are commonly used to prevent angioedema episodes. Anaesthesia management and perioperative implications have been recently commented by some authors [3]. Airway oedema and hypovolemic shock due to the tissue leak of fluids are especially significant in the perioperative period, chall...

OBJECTIVE To analyze the surgical outcome of traumatic lumbosacral spondylolisthesis treated using posterior lumbar interbody fusion, and help spine surgeons to determine the treatment strategy. METHODS We reviewed retrospectively five cases of traumatic lumbosacral spondylolisthesis treated in our hospital from May 2005 to May 2010. There were four male and one female patient, treated surgic...

Background and Aim: Degenerative lumbar scoliosis is a spinal deformity resulting from advanced disc degeneration and facet arthropathy. Given the inconclusive available literature and lack of high-quality data supporting the role of minimally invasive surgical management of degenerative lumbar scoliosis, this review intends to highlight and compare the various viable minimally invasive surgica...
متن کاملGuidelines for the performance of fusion procedures for degenerative disease of the lumbar spine. Part 17: bone growth stimulators and lumbar fusion.
There have been a number of randomized studies supporting the use of ES for the promotion of bone healing following lumbar fusion. All of the published studies have methodological flaws that prevent the studies from providing Class I medical evidence. There is, however, Class II and III evidence to support the use of direct current stimulation or CCS for enhancing fusion rates in high-risk pati...

The procedure of anterior lumbar interbody fusion (ALIF) is commonly performed on patients suffering from pain and/or neurological symptoms associated with disorders of the lumbar spine caused by disc degeneration and trauma. Surgery is indicated when prolonged conservative management proves ineffective. Because an important objective of the ALIF procedure is solid arthrodesis of the degenerati...
